What causes pulmonary hypertension?

Pulmonary hypertension may be caused by diseases of the heart and the lungs, such as chronic obstructive pulmonary disease (COPD), emphysema, failure of the left heart ventricle, HIV, blood clots, sarcoidosis, or advanced liver disease! on! Any Suggestions here?
